OSHA finds 15 violations in grain bin death

A federal agency has slapped an ethanol manufacturer with 15 health and safety violations, and recommended the company be fined $140,000 after an investigation into the death of a worker who died when he was buried by corn in a grain silo April 19 at the company's Milton, MN, plant.
